The Trustee requires that a cover letter addressing each recommendation item from the plan evaluation is submitted with each proposed SOC. After submission, create a docket entry in ECF: Bankruptcy → Notices → Notice of Submitting.
Chapter 13 Plan
Arizona Local Rule of Bankruptcy Procedure 2084-4 requires use of a specific plan form, Local Form 2084-4:

Business Operating Statement – Submission Addresses
For debtors without an attorney, the operating statement must be hand delivered or mailed to the court. For debtors with an attorney, follow the debtor-attorney agreement. The court will provide a copy to the Trustee's office.
